Healthcare is one of the most sensitive issues concerning citizens, as
it directly affects the quality of life. As such, attention to the
management of public funds to this sector takes on a special importance.

The Ministry of Health (MoH) is one of the three pillars of the public
institutions that participate in the Albanian healthcare system. Along
with the Institute of Health Insurance (IHI) and the local governments,
they care about providing a more effective health service.

The budget allocated to this Ministry has been increasing, but it has
not always had the same importance in the Government Budget. In
2003-2005, the MoH budget grew by about 9% and in 2005 it received the
largest share of funds ever allocated to this sector from the Government
Budget (14.1%).

In the following years, the budget of the MoH continued to grow, but its
share in the Total Budget began to fall. It fell from 14.1% of the
total funds in 2005 at 9.5% in 2008, a fact that describes a reduction
of the importance of this sector in the Government policies.

In 2009, a significant increase of the funds is observed (15.8% compared
to 2008), and a slight decrease in 2010. In 2011, both MoH budget
(14.5% compared to 2010) and its share in the Total Budget have
increased.

The Ministry of Health achieves its goals through four programs:
“Administration, Management, Planning”, “Primary Care”, “Secondary Care”
and “Public Health”.

Among these programs, most of the funds go to the Secondary Care
program, including the hospital service. This program has had a large
funding increase, attaining the maximum level in 2009 (16.3 billion
ALL). In 2010, these funds fell by 6% compared to 2009; however, they
were 40% higher than in 2005.

We should emphasize the fact that a large part of the MoH funds are
administered by the Institute of Health Insurance (IHI). Since 2007, IHI
is responsible for providing Primary health care services and since
2009 also for the Secondary care. To achieve these functions, MoH
transfers the funds from the Government Budget to the IHI. These changes
are altering the MoH traditional role from direct management of health
services towards focusing more on policy, plans, development strategies
and regulatory of the health system.
